For your convenience, here are several forms which you can print out and fill in before you visit Foot & Ankle Specialists of West Michigan.

Insurance and Payment Policy

Foot & Ankle Specialists are pleased to announce that we participate with the majority of health care insurance plans. For your convenience, we will submit the charges to your insurance for you.

For our HMO, PPO and managed care patients, we do remind you that it is your responsibility to obtain authorization from your primary care physician for your treatment.

By our contract with your insurance, we are to collect your co-pay at the time of service. Please be prepared to make this payment.

We ask for an administrative fee for completion of some types of disability forms.

We understand our patients' concern at the cost of health care. The cost of care should not create a hardship; we encourage our patients to discuss any questions concerning their care with our physicians or staff members.
